	Registration for the Lodge Council Camp Service Weekend  
 

	Every registrant will need to provide the following information during the registration process:  
 

	
		Name as registered with NCAC (first and last) 
	
		Address (full mailing address including ZIP 4) 
	
		Age Category 
	
		Gender 
	
		BSA ID Number (number on your NCAC Registration Card) 
	
		Unit Type &amp; Number 
	
		Chapter/District 
	
		Transportation to and from the event 
		
			
				If you are not driving yourself; the name of your driver will be requested 
		
	
	
		Any diet or religious food restrictions 
	
		Medical condition(s) requiring medication(s) 
	
		Certify that you have read the Amangamek-Wipit Lodge Participant Code of Conduct and Statement of Understanding and agree to abide by the conditions contained therein 
	
		If you have NOT read the Code of Conduct, use the link above to open and print the document 
	
		Bring the signed Code of Conduct with you to the Ordeal 


	


	Current Order of the Arrow members will need to answer the following during the registration process:  
 

	
		If you are an Ordeal member will you be participating in the Brotherhood Conversion 
		
			
				If yes, the date of your Ordeal (must be at least 10 months prior to the event) 
		
	
	
		Was your ordeal completed with Amangamek-Wipit 
	
		If no, the name of the BSA Council where it was completed will be requested 
	
		Will you be participating as an Elangomat (you must have prior approval of the Chapter Chief and/or Adviser; if you have been asked to participate as an Elangomat, you will select &ldquo;Yes&rdquo; 
	
		Are you registering for the &ldquo;Feast&rdquo; only 


	


	Notice to all candidates  
 

	All ordeal candidates that have been elected during the election period (November 1, 2013 to May 31, 2013) should have received a written notice confirming their candidacy and an invitation to attend the upcoming ordeal for their chapter.  
 

	If you are unable to participate in your chapter ordeal you should contact your chapter adviser to make arrangements to attend one of the other listed ordeals. You must make arrangements in advance to attend an ordeal in outside of your chapter/district.  
 

	


	Notice all Order of the Arrow members  
 

	All active Order of the Arrow members are encouraged to register. An active Order of the Arrow member is:  
 

	Currently registered with BSA and  
 

	2013 lodge dues ($12.00) are paid  
 

	If your dues are not current you will need to pay them on site (Dues Form).  
 

	


	BSA Health &amp; Medical Record Form required for all lodge/chapter events  
 

	All Order of the Arrow members/candidates who attend the Ordeal MUST complete parts A &amp; B of the BSA Annual Health and Medical Form #680-001 (for events under 72 hours in length).  
 

	All medications need to be in the original containers with the Scouts&#39; name, dosage requirements (containers will be returned), and with the correct amount of Medication for the weekend. This includes over the counter drugs with a signed note from Parent/Guardian with dosage and instructions and times to be taken. Place all containers in a zip-locked bag labeled with the Scouts&#39; name.  
 

	BRING THE COMPLETED HEALTH &amp; MEDICAL RECORD FORM WITH A PHOTOCOPY OF BOTH SIDES OF YOUR INSURANCE CARD TO THE ORDEAL, IF THE FAMILY DOES NOT HAVE MEDICAL INSURANCE STATE &quot;NONE&quot;  
 

	IMPORTANT NOTICE  
 

	If you arrive without your Health &amp; Medical Record Form you will not be allowed to participate in the ordeal and will not receive a refund.  
 

	


	Cancelation Policy  
 

	Refunds for this event may be granted (less 15% handling fee April 26, 2013 (e-mail merchantj@verizon.net).  
 

	Fees cannot be credited toward another event. This policy is true for any event with fees over $25.00 per participant/registrant unless stated otherwise.
