	Every registrant will need to provide the following information during the registration process:  
 

	
		Registration category 
		
			
				Lodge officer/adviser; 
			
				Chapter officer/adviser; or 
			
				OA member
		
	
	
		Address (full mailing address including ZIP + 4) 
	
		Age Category 
	
		Gender 
	
		BSA ID Number (number on your NCAC Registration Card) 
	
		Chapter/District 
	
		Unit Type & Number
	
		Transportation to and from the event 
		
			
				If you are not driving yourself; the name of your driver will be requested 
		
	
	
		If you have NOT read the Code of Conduct, use the link above to open and print the document 
	
		Certify that you have read the Amangamek-Wipit Lodge Participant Code of Conduct and Statement of Understanding and agree to abide by the conditions contained therein 


	


	Lodge/Chapter officers & advisers will need to answer the following during the registration process:  
 

	
		Lodge Leadership 
		
			
				Youth officer 
				
					
						Lodge chief, lodge vice chief, lodge area chief, lodge area vice chief, or lodge committee chair

	Notice all LLDC participants  
 

	All active lodge and chapter officers/advisers encouraged to register. An active Order of the Arrow member is:  
 

	Currently registered with BSA and  
 

	2013 lodge dues ($12.00) are paid  
 

	If your dues are not current you will need to pay them on site. (Dues Form) 
 

	


	Candidacy for Lodge Office  
 

	Members interested in running for lodge office should complete the Petition for Lodge Office prior to the LLDC. 
 

	
		All candidates MUST be:
		
			
				Under the age of 21 during the entire term of office,
			
				Current dues paid with Amangamek-Wipit Lodge and
			
				Currently registered with the Boy Scouts of America National Capital Area Council
		
	
	
		The petition MUST be signed by:
		
			
				The candidate’s parent/guardian, if the candidate is under 18 years of age and
			
				The candidate’s unit leader (i.e. Scoutmaster, Coach, Crew Adviser, or Skipper)
		
	
	
		Completed forms should be mailed or delivered to the lodge adviser
