Registration for the Amangamek-Wipit Vigil Induction Weekend
Every registrant will need to provide the following information during the registration process:

Address (full mailing address including ZIP + 4)

Age Category

Gender
-
BSA ID Number (number on your NCAC Registration Card)

Unit Type & Number
-
Chapter/District
-
Transportation to and from the event
-
If you are not driving yourself; the name of your driver will be requested

Vigil Candidates Should not drive themselves to the vigil induction
-
Any diet or religious food restrictions
-
Medical condition(s) requiring medication(s)

Certify that you have read the Amangamek-Wipit Lodge Participant Code of Conduct and Statement of Understanding and agree to abide by the conditions contained therein

If you have NOT read the Code of Conduct, use the link above to open and print the document

Bring the signed Code of Conduct with you to the Ordeal
Current Vigil Honor members will need to answer the following during the registration process:
Indicate the area of participation you will be assisting with during the vigil induction

General Support
-
Ceremonialist
-
Guide for Vigil Candidate; provide the name of the vigil candidate you will be guiding
Note: BSA Policy will not allow you perform more that one of the following roles:

Driver for Vigil Candidate; provide the name of the vigil candidate you will be driving

“Over night watch”
Notice to all Vigil Candidates
All vigil candidates that have been selected should have received a written notice confirming their candidacy and an invitation to attend the upcoming vigil induction.
If you are unable to participate in your in one of the vigil inductions you should contact vigil induction adviser to make arrangements for an alternate vigil induction ceremony. All vigil candidates are strongly encouraged to participate in one of the two scheduled vigil inductions.
Notice all Vigil Honor members
All active Vigil Honor (Order of the Arrow) members are encouraged to register. An active Order of the Arrow member is:
Currently registered with BSA and
2013 lodge dues ($12.00) are paid (Dues Form)
If your dues are not current you will need to pay them on site.
BSA Health & Medical Record Form required for all lodge/chapter events
All Vigil Honor members/candidates who attend the vigil induction MUST complete parts A & C of the BSA Annual Health and Medical Form #680-001 (for events under 72 hours in length)
BRING THE COMPLETED HEALTH & MEDICAL RECORD FORM WITH A PHOTOCOPY OF BOTH SIDES OF YOUR INSURANCE CARD TO THE VIGIL INDUCTION, IF THE FAMILY DOES NOT HAVE MEDICAL INSURANCE STATE "NONE"
IMPORTANT NOTICE
If you arrive without your Health & Medical Record Form you will not be allowed to participate in the vigil induction and will not receive a refund.
Cancelation Policy
Fees cannot be refunded or credited toward another event. This policy is true for any event with fees over $25.00 per participant/registrant unless stated otherwise.
