Registration for the Zekiah Chapter Ordeal Weekend
Every registrant will need to provide the following information during the registration process:

Name as registered with NCAC (first and last)

Address (full mailing address including ZIP 4)

Age Category
-
Gender
-
BSA ID Number (number on your NCAC Registration Card)

Unit Type & Number
-
Chapter/District
-
Transportation to and from the event
-
If you are not driving yourself; the name of your driver will be requested

Any diet or religious food restrictions
-
Medical condition(s) requiring medication(s)

Certify that you have read the Amangamek-Wipit Lodge Participant Code of Conduct and Statement of Understanding and agree to abide by the conditions contained therein

If you have NOT read the Code of Conduct, use the link above to open and print the document

Bring the signed Code of Conduct with you to the Ordeal
Current Order of the Arrow members will need to answer the following during the registration process:

If you are an Ordeal member will you be participating in the Brotherhood Conversion
-
If yes, the date of your Ordeal (must be at least 10 months prior to the event)
-
Was your ordeal completed with Amangamek-Wipit
-
If no, the name of the BSA Council where it was completed will be requested
-
Will you be participating as an Elangomat (you must have prior approval of the Chapter Chief and/or Adviser; if you have been asked to participate as an Elangomat, you will select “Yes”

Are you registering for the “Feast” only
Notice to all candidates
All ordeal candidates that have been elected during the election period (November 1, 2012 to May 31, 2012) should have received a written notice confirming their candidacy and an invitation to attend the upcoming ordeal for their chapter.
If you are unable to participate in your chapter ordeal you should contact your chapter adviser to make arrangements to attend one of the other listed ordeals. You must make arrangements in advance to attend an ordeal in outside of your chapter/district.
Notice all Order of the Arrow members
All active Order of the Arrow members are encouraged to register. An active Order of the Arrow member is:
Currently registered with BSA and
2012 lodge dues ($12.00) are paid
If your dues are not current you will need to pay them on site (Dues Form).
BSA Health & Medical Record Form required for all lodge/chapter events
All Order of the Arrow members/candidates who attend the Ordeal MUST complete parts A & C of the BSA Annual Health and Medical Form #680-001 (for events under 72 hours in length).
All medications need to be in the original containers with the Scouts' name, dosage requirements (containers will be returned), and the correct amount of medication for the weekend. Please place all containers in a zip-locked bag labeled with the Scouts' name.
BRING THE COMPLETED HEALTH & MEDICAL RECORD FORM WITH A PHOTOCOPY OF BOTH SIDES OF YOUR INSURANCE CARD TO THE ORDEAL, IF THE FAMILY DOES NOT HAVE MEDICAL INSURANCE STATE "NONE"
IMPORTANT NOTICE
If you arrive without your Health & Medical Record Form you will not be allowed to participate in the ordeal and will not receive a refund.
Cancelation Policy
Refunds for this even may be granted (less 15% handling fee) for the following reasons: sickness (with physician's statement), relocation, or death in the family.
Requests must be in writing and received by October 12, 2012 (e-mail to oazekiah@yahooo.com).
Fees cannot be credited to another event. This policy is true for any event with fees over $25.00 per participant.
